Welcome to our new patients!

If you have scheduled an appointment to be seen by one of our surgeons in the office, we have provided the New Patient Questionnaire form to expedite your registration process. Once you have downloaded the form, please print it, fill it out, sign it and bring it with you when you come to our office.

A few helpful reminders for the day of your appointment

Please call our office before you travel to your appointment to ensure the surgeon will be available at the scheduled time. Unforeseen circumstances may arise that could cause a delay in your appointment time. We will do our best to see you at your originally scheduled time.
Please bring a list of your current medications
Bring your insurance card, any applicable co-pay and referral (if required by your insurance company).
Bring any studies as requested by our office.
